A caregiver responsible for a group of children ages 7 months- 12 years old was unaware of the number of children responsible for. The caregiver stated eight were present, however nine were present in the room.
Personnel Records were evaluated for five staff members and the following was determined: Two staff members needed a statement indicating they have received the personnel and operational policies. One staff member needed documentation showing how they meet the minimum educational requirement. One staff member needed documentation showing orientation training was completed.
Records were evaluated for six children enrolled and the following was determined: Three needed updated immunization records. Two needed an address for the emergency contact. Two needed a health statement. One needed a special needs statement. One needed a food allergy statement.
The sign in and out log was reviewed and showed 1 of 15 children were signed in. Note: This was corrected at inspection.
The menu stated the children would be served the following: Chicken, Bananas, Sweet Potato Fries, and Milk The children were served chicken, rice, cucumbers, and milk. Note: This was corrected during inspection.
-The door to the men's restroom was off the hinge. I observed nails sticking out of the area. -Tarp was observed protruding from the loose fill surfacing, presenting a tripping hazard.
Based on the information obtained, it was determined a caregiver responsible for a group of children ages 3-5 years old did not maintain supervision when a child was able to pull out another child's hair.
Four staff records were reviewed and the following was determined: 1 needed a record of training hours. 1 needed proof of educational qualifications. 2 needed photo identification. 3 needed proof of background check request.
I observed a three year old unsupervised in the after schoolers classroom. One caregiver was observed caring for two different groups of children. One group consisted of three children ages 13 months, 13 months, and 4 years. The other group consisted of 7 children ages 2-4 years old.
