The operations emergency prepoaredness plan was reviewed and observed to be missing the following; The first responsibility of staff in an emergency evacuation or relocation is to move the children to a designated safe area or alternate shelter known to all employees, caregivers, parents, and volunteers; (B) How children will be evacuated or relocated to the designated safe area or alternate shelter, including specific procedures for evacuating and relocating children who are under 24 months of age, who have limited mobility, or who otherwise may need assistance in an emergency, such as children who have mental, visual, or hearing impairments; (C) The staff responsibility in a sheltering/lock-down emergency for the orderly movement of children to a designated location within the center where children should gather; (D) An emergency evacuation and relocation diagram as outlined in ?746.5207 of this title (relating to Must I have an emergency evacuation and relocation diagram?); (E) Name and address of the alternate shelter away from the center you will use as needed; and (F) How children in attendance at the time of the emergency will be accounted for at the designated safe area or alternate shelter. The emergency telephone number that is on file with us; and (B) How you will communicate with local authorities (such as fire, law enforcement, emergency medical services, health department), parents and CCR How your staff will evacuate and relocate with the essential documentation including: (A) Parent and emergency contact telephone numbers for each child in care; (B) Authorization for emergency care for each child in care; and (C) The child tracking system information for children in care; (4) How your staff will continue to care for the children until each child has been released; (5) How you will reunify the children with their parents as the evacuation, relocation, or sheltering/lock-down is lifted.

Staff in the infant and prek rooms were not aware of the ages of children in their care. In the infant room, when asked, the caregiver stated the children were 9-13mo. When asked later for the individual ages to determin specified age group(SAG), the caregiver then stated the youngest in care was 4mo and the oldest was 20 months. In the prek room. The caregiver stated the children were 3-5yrs. When asked for individual ages of children to determin SAG, the caregiver stated there were 10-15 3yr olds in care and 5 5yr olds. Staff present continuned to provide in consistent information on the ages of children in their care. One caregiver stated there were no 5yr olds in care. NOTE: The birthday wall posted only listed the ages of 14 children

The directors record was reviewed and current training in allergy response and hazardous materials was not available for review. NOTE: The last documents training availalbe was observed to be completed 6/2022.
Of the 9 staff records reviewed, 8 did not have training in responding to food allergy reactions. 9 did not have training in handling hazardous materials.
Two seat belts were observed to be stuck and could not be buckled.
A caregiver was observed changing 2 diapers and did not sanitize the diaper changing table in between.

2 sheets were observed to be loose. One sheet was observed to be tied in knots under the mattress causing the ends of the mattress to bend. NOTE: Corrected at inspection. The walking infant was removed from the crib and placed on a mat. The other mattress/sheet was removed from the room.
Cribs in the infant room were not observed to have enough space. Three infants in cribs were observed to be able to touch each other from their cribs.
Written feeding instructions for a 4 mo old not yet on table food was not available for review.

A 4 month old infant was observed to be in a crib with a blanket and a bottle of milk.
3 infants between the ages of 14mo-16mo were placed in cribs with bottles.

A bag of carrots and an avacado was observed to be open and uncovered in the refrigerator.

Of the 11 children's records reviewed, 6 did not have an address listed for the emergency contact, 1 did not have a name listed, and 2 children did not have anyone listed as an emergency contact.
First aid kit was observed to be missing cotton balls, scissors, tweezers, and a thermometer. NOTE: Corrected at inspection, all missing items were placed on the van.
Documentation that two caregivers received orientation was not available for review.
Emergency medical information was not available for one child being transported.

Upon arrival a caregiver was observed running into a classroom. In the classroom, another caregiver was observed supervising 2 groups, According to the director, the caregiver was asked to stand in the doorway and watch both groups so they could go to the restroom.
Fire drills have not been conducted monthly or within 3 minutes. The last fire drill conducted was July 1st and lasted 4 minutes.
The vehicle that will be used for transportation did not have a first aid kit
