An infant was observed in a restrictive device longer than fifteen minutes during this inspection and was brought to the attention of the caregiver in charge. Note: The child was observed in the restive device for approximately 47 minutes. The director removed the child from the restrictive device upon arrival.

Records were evaluated for ten children enrolled and the following was determined: Two children needed the telephone number of the school they attend. Two children needed the date of admission. Two children needed a health statement signed by a physician. One child needed an emergency contact other than the parent. Two children needed emergency care information such as physician name, address, and phone number. One child needed authorization for emergency care. A food allergy emergency plan is needed for one child with a diagnosed food allergy. A parent signature is needed on the food allergy emergency plan for one child.
I observed a bottle of cleaning solution near the handwashing sink in the toddler classroom. Note: This was corrected during inspection.
During this inspection, one caregiver did not have documentation of current pediatric CPR training.
