I observed 24 children in a classroom including infants with one caregiver. The youngest child in the room was 17 months. The oldest child in the room was 5 years old. Note: A caregiver showed up to relieve the other teacher and the children were all put back in the correct classes, according to ages.
I observed a caregiver leave the room with 7 children ages 18 months-23 months for any extended time frame. The caregiver was also on a personal device. Note: Supervision was discussed, however the citation was recited. I observed two sharp objects accessible to children in the kitchen. I also observed a caregiver leave a classroom with 9 children in care, ages 2-3 years of age.
I observed ten children ages 18 months through 23 months and a 3 year old being supervised by one caregiver in the rear classroom. The maximum number of children in this age group that one caregiver may supervise is nine.
I observed while outdoors standing water, tarp that covers the play equipment in need of repair, the gate to the the electric box open and the back gate open.

During the inspection I observed a electrical box that was lifted out of the wall exposing wires, Broken fence boards on the playground as well as a wooden deck with the astro turf covering ripped and torn, broken plastic toys as well as debris on the ground.
During the walkthrough I observed a caregiver without a cleared eligible background results caring for 22 children ages 2-3 years old.
