One caregiver did not have CPR training and the other caregiver had online CPR training.

The director had 15 children in care specified age groupe was 2 years old, the director was over by four children.

The fence to the left of the operation on the playground was not 4 feet tall, it was 3 feet 4 inches.

Doing naptime two children were in another classroom not being supervised by the caregiver.

The director stated she had 14 children when I counted they were 15 children in care.

A child was on their tablet doing the nap time period.

A Little Tykes type of playground play equipment has deteriorated on the play surface causing the plastic to rip and become sharp and jagged. Wooden borders in the playground need replacing.

The 5'6" slide does not have the required 6 foot use zone around the perimeter of the slide and overlaps the use zone of another piece of equipment.

The operation has 3 fire extinguishers rated at 2A--10BC.

The backside of the center facing and exposed to the playground is a painted surface with chipping paint and has 2 broken boards in need of replacing. The exterior boards are accessible to children.

The center's emergency prepardness plan needs updating to include language in how caregivers will safely move children under 24 months old to the relocation site.
