There was Lysol wipes in the classroom accessible to children. Note: Corrected at inspection.

There was evidence to support that this standard is deficient. The caregiver stated she did not check to see if the child needed to be changed prior to dinner transition.

The most recent fire inspection was conducted on 6/16/2021.

On the playground there is Monkey bars and a slide without the proper loose fill.

Director has 21 hours of annual training for 21-22 school year.

There is a sign saying "STOP parents not allowed at a certain point. Note: The sign was removed during the visit.

During the inspection a 10 month old was observed in the crib with a pacifier clip. This was corrected at the inspection when the caregiver removed the clip.

In the infant classroom 3 bottles were not labeled with the child's first and last name. This was corrected when the caregiver labeled the bottles.

A 6 month old was observed sleeping in a swing with a blanket.

The baseboard in the schoolers classroom was chipping.

One caregiver was supervising a group of five students, 6 month to 14 months putting the caregiver out of ratio by one. One caregiver was supervising a group of 10 students, 18 mo to 2 years old putting the caregiver out of ratio by one.

One infant in care did not have the required daily report.

Bottles in the infant class were observed without labels.

Three school age students were in a classroom unsupervised while the staff member was located at the front desk.

The written feeding instruction for one infant was lacking a recent update. The last update was 3/7/2021.

Four of five infants were observed sitting at the lunch table watching tv on a phone.

The beds located in the infants classroom were lacking labels for students.

Two out of ten records were lacking the required school information.

The toilet in the pre-k classroom was observed to have excrement on it.

Five out of ten files were lacking the required emergency contact address.

The school age playground was lacking the required 9 inches of sand.

Staff records were not available during the inspection.

The lighting in the toddler and preschool room was insufficient. Children in the classrooms could not be seen upon entering.
