Three mushrooms were observed on one of the swing sets on the playground. This was corrected during the inspection when I observed the director remove the mushrooms and place them in the trashcan.

Based on the information obtained during the investigation conducted by DFPS the infant caregiver admitted to allowing an infant to sleep in a bouncer and denied placing the infant in the crib.

Based on the information obtained during the investigation conducted by DFPS it was determined that there were five infants being supervised by one caregiver. Based on the information obtained the caregiver was supervising two four-month-olds, one three-month-old and one fourteen-month-old. The other infants age was undetermined however according to minimum standards the specified age group would be four months.
