Three caregivers who were hired on 12/2023, 4/2024 did not have the pre-service training certificate available for review before being left in charge of the classroom during the inspection.

A caregiver from 12mo classroom were observed using the tablet to complete their school homework while supervising children during nap time.

It was observed the infants from the Infants Classroom did not have their hands washed after their diaper changed. This was corrected when the caregiver was made aware and washed the infants hands during the inspection.

Two employees did not have the CPR/FA training certificates available on file for review during the inspection

One staff member with "Provisional" background check status was observed present and supervising 10 children ages 2 years old alone during naptime on the inspection date 4/15/2024.

Based on the information gathered, there's sufficient evidence the caregiver did not serve the food on a plate or napkin in a sanitary manner.

One employee with "Inactive" background check status was confirmed present and was on the field trip with the children in care. This was corrected when the Background check for the employee was resubmitted during the inspection.

Based on the information obtained from the investigation, the incident report completed for the incident that occurred to a child in care was not completed properly when the information entered was incorrect.

The completed admission information for one child was not available for review during the inspection.

Although the operation has cribs available for infants to sleep in, they were obseved not being used and two infants ages 3 months old and one 6 months old were observed sleeping on the floor during the inspection.

Based on the information obtained from the investigation, it is determined that self-control was not being maintained by a caregiver when interacting with children in care.

Emergency Safety Drill documentation was reviewed showing Fire drill was last conducted in April 2022

The medication administration traininng documentation for two employees are not available for reviewed.

Annual training documentation for three employees were not available for review during the inspection; One employee who was hired in January 2021 missing 13 hours of annual training; One employee who was hired in February 2021 missing 7 hours of annual training; One employee who was hired in 6/2/21 missing 18 hours of annual training. One employee who was hired in May 2021 did not have annual training documentation available for review.

First aid certification for six employees were not available for review during the inspection.

CPR certification for six employees were not available for review during the inspection.

The transportation training documentation for the director and two employees were not available for review.

List of current employees was observed not being posted during the inspection walkthough; This was corrected when list of updated employees was updated and posted.

The director personnel file was observed missing the driver license; notarized affidavit One employee did not have proof of highschool diploma or equipvalency documentaton available for reivew.

Seven infants from Infant 1 classroom did not have their most current feeding instructions available for review.

The annual Sanitation Inspection Permit was observed expired during the inspection.

The medication administering for two employees were not available to review.

The transportation training for one employee was not available to reviewed.

The annual Fire Inspection Permit was observed expired during the inspection.

Out of the ten staff files reviewed, seven of the files were observed to be missing a notarized affidavit.

One of the two transportation vehicles does not have a working child safety alarm.

The driver's license of the director is expired.

The caregiver in the toddler room was unable to tell me the ages of the three children she had in care and she was unaware which child was the oldest/youngest.

The operation did not have documentation that a gas leak test was conducted on the operation.

One of the infants was sleeping in an unlabeled crib. This was corrected when the infant caregiver labeled the infant's crib with her name.
