The infant room was observed to be out of ratio during the follow up inspection. Upon entering the room, there was one caregiver with 8 infants. Note: 2 caregivers entered the classroom during the inspection.

The operation did not have documentation of a completed food allergy plan for 2 children's files.

Two infant classrooms did not maintain ratios during the inspection. In one room the child to caregiver ratio was 7:1. In another room the ratio was 9:1. Note: A caregiver was added to each classroom during the inspection.

Based upon the findings from the DFPS Investigation, there was sufficient evidence to support the operation did not foster appropriate independence in children by not allowing children to make their own choices and decisions.

Based upon the information obtained, there was sufficient evidence to support a caregiver used an inappropriate tone with children.

Children in care had not been signed in to the tracking system during the inspection. Note: The provider corrected this at inspection by checking each child in.

Infants were observed sleeping with pacifiers which contained an attachment piece to their clothing. This posed a choking hazard. Note: The caregiver removed the attachment fromt he pacifier during the inspection.

Based upon the information obtained, the operation has not completed incident reports.

There was inadequate loose-fill material in the play area.

A caregiver pulled a child by the wrist during the inspection.

An electrical outlet was not covered in the classroom during the inspection. Note: The provider placed a cover over the outlet during inspection.

The use zone for stationary play equipment does not extend 6 feet around the perimeter of the equipment.

2 diaper changing mats had plastic garbage bags covering them and cannot be properly cleaned. Note: The provider removed the plastic bags during the inspection.

Two out of ten child records reviewed is missing the health statement on file.

Based on the DFPS investigation, there is sufficient evidence to suggest that prohibited punishment occurred in the operation.

In the outdoor area by the entrance door the wooden floor was lifting. In the outdoor area there was a fence board that was broken. Note: The wooden floor and fence board was repaired during the inspection.

Based on the investigation and information obtained the operation is not in compliance. Parents are not being allowed in the classrooms to observe their children.

I observed 9 children ages 15-20 months sitting at toddler tables. Straps were installed and used for one seat.
