Kids of Valor Academy

A caregivers purse was observed to be sitting on a shelf on top of children's toys and activites in the 3-4yr group. The purse was in reach and accessible by children. NOTE:Corrected at inspection, the caregiver removed the purse from the room.

Based on information obtained during the investigation, there is sufficient information to support, infants in care do not go outside.

Director and caregivers must operate in compliance at all times.

Caregiver was observed walking in and out of the classroom during naptime.

A child received medical attention after accidentally seeping a small amount of milk and was transported to ER. This incident happened 1/18/2024 during lunch time and was not reported to Licensing. For this citation there will be an administrative penalty of a flat rate of $500.00.

Licensing did not observe any training for caregiver who was in charge of victim when the ambulance was called. This training is Orientation prevention and responding to allergic reaction.

One new staff name was missing on the current employee list. This was corrected when director added her name to the list and then posted the list.

Three children did not wash their hands after naptime before eating their snack.

Based on the video clip that was forwarded to licensing a staff did pick up a child roughly during naptime and another staff did push another child back onto the cot.

The health inspection is not current.

Fire Marshall inspection is not current.

Current employee list was missing one staff. This was corrected during inspection when the director updated this posting.

Fire drill for the month April was not completed and documented.

One staff record did not have current first aid and Cpr training on file.

Our system is showing that background check for the amount of $10 has not been paid. This was corrected during inspection when director paid online.

One electrical outlet in the area used by four years and up was not covered. This was corrected during inspection.

I observed holes on the wall and electrical outlet pulling from the wall in one classroom that was not currently used by children.

A child on high chair did not have safety straps on. This was corrected when caregiver removed the child from highchair.

There was a dead roach found in the infant classroom.

Upon request, the caregiver in charge was unable to provide records to licensing. Note: This was corrected during the inspection when the owner provided the requested records to licensing.

Documentation showing proof of education was still not available for me to review for one staff file during the follow up.

The child safety alarm in the van is not being maintained. Upon testing the alarm multiple times during the inspection it made no sound.

1 out of 4 staff records evaluated did not contain proof of education.

2 out of 4 staff records evaluated did not contain completed orientation.

A 12 month old was combined with older children during nap time. The oldest child in the room was 12 years old. Note: The provider moved the infant to the infant room during the inspection.

Multiple outlets did not contain an outlet cover during the inspection. Note: The provider corrected this during the inspection.

A caregiver was observed alone outside with 4 children.The operation has not obtained an eligibility notification from CBCU. The caregiver left the operation during the inspection.

Disinfectant spray was left on the second shelf in the toddler classroom in reach of children. Note: The disinfectant spray was removed during the inspection.

Based upon the information obtained from the DFPS Investigation, A child fell while in care and the caregivers did not contact EMS causing the child to have an untreated broken collar bone.

Based upon the information obtained from the DFPS Investigation, caregivers failed to practice good judgment when they picked up a child by the arms after she had fallen out of a chair and continued to pick the child up by the arms instead of under the arms.

Based upon the information obtained from the DFPS Investigation, the operation failed to notify CCR of an injury a child received while in care which required treatment by a Health-Care Professional.

Based upon the information obtained from the DFPS Investigation, the operation failed to notify parents when a child fell and was injured.

Based upon the information obtained from the DFPS investigation a child fell twice and the operation did not complete an incident report.

Based upon the information obtained from the DFPS Investigation, caregivers used phones while they were responsible for children in care.
