The emergency preparedness plan is missing the following: 1. The name and address is missing from the emergency preparedness plan. 2.How your staff will continue to care for the children until each child has been released. 3. How you will reunify the children with their parents as the evacuation, relocation, or sheltering/lock-down is lifted. Note: The Director added the missing procedures to the plan during the inspection.

An 8-month-old infant was observed asleep in a bouncer with a milk bottle.

A child enrolled does not have an immunization record on file at the operation. Note: The Director obtained a copy of the child's immunization record during the inspection and placed it in the child's file.

The operation does not have documentation that a recent fire inspection have been conducted. The last inspection conducted was on 10/27/2020.

The building and grounds checklist has not been completed on a monthly basis. The checklist was last completed in October. Note: The Director completed the checklist for today during the inspection.

A child that has a diagnosed food allergy and requires an EpiPen does not have a food allergy plan on file at the operation. Note: The Director received the food allergy plan from the child's parent. The plan is signed and dated by the healthcare professional and parent.

A child's record reviewed was missing the following form the admission form: 1. Permission for transportation, 2. A statement of the child?s special care needs. This includes, but is not limited to, allergies, existing illness, previous serious illness and injuries, hospitalizations during the past 12 months, and any medications prescribed for continuous, long-term use
