Personnel records were evaluated for eight staff members and the following was determined: Documentation showing one caregiver meets minimum educational requirements is needed. A notarized affidavit is needed for two staff members. Background check letters of eligibility is needed for eight staff members. Three staff members need photo identification.

The most recent sanitation inspection was not available for review during this inspection.

The caregiver in the school age classroom was unaware of the number of children present. The caregiver stated nine children were present, however eleven were present in the room.

The sign in and out logs were reviewed during today's inspection. Ten of eleven children were signed in the after school classroom.

The sign in and out logs were reviewed during today's inspection. The sign in and out log for the infant classroom showed four of nine infants were signed in. Six of ten children were signed in the after school classroom.

A ten month old was observed seated in a high chair. The safety straps were not fastened.

The operation has a current infant capacity of nine. During today's inspection a total of ten infants ages 3 months, 3 months, 3 months, 7 months, 7 months, 7 months, 7 months, 10 months, 11 months, and 15 months. The operation was over the infant capacity by one infant.

A caregiver was observed lifting children under two years old by one arm during this inspection.

A caregiver responsible for a group of children ages 3 months-18 months was unaware of the number of children present. The caregiver stated six children were present, however nine were counted in the area. A caregiver responsible for a group of children ages 3-5 years old. The children were counted and the caregiver was informed sixteen were present.

The sign in and out log was reviewed in the Pre-K classroom and showed 11 of 16 children signed in. The sign in and out log was reviewed in the Two's and Three's classroom and showed 13 of 14 children signed in. Note: This was corrected during inspection.

Upon arrival, one caregiver was caring for five infants ages 3 months, 3 months, 7 months, 7 months, and 9 months. The specified age group is 0-11 months. The maximum number of children one caregiver may supervise for this age group is 4. The caregiver was over ratio by one child.

Updated immunization records are needed for two children in care.

One staff file reviewed needed training in the following topics: -Child Maltreatment -Emergency Prepardness -Administering Medication -Understanding Building and Physical Premises Safety -Handling, Storing, and disposing hazardous materials.

I observed four children sitting in seats that required safety straps. The safety straps were not used.

Seven personnel files needed documentation showing how the employee meets the education qualifications.

Infant feeding instructions were unavailable for one child in care.

A four month old was observed laying in a crib. A pacifier with a stuffed animal was attached to the infant.

One caregiver was observed caring for five children age 4 months-17months. The specified age group was 0-11 months. The classroom was out of ratio by one child.

Eight children's records were reviewed and the following was determined: One needed release persons. Two needed an address for the emergency contact. Two needed an updated immunization record.

A group of nine children, age three, were observed unsupervised in the schoolers room. A group of children age 18 months-29 months were observed unsupervised in the toddler room.
