During this investigation it was determined the director allowed a staff member to transport children before completing transportation training.
During this investigation it was determined three children ages 4, 4, and 11 years old were not using safety belts while being transported.
During this investigation it was determined a driver who was transporting children did not complete transportation safety training.
Based on the information obtained, it was determined an eleven year old was allowed to ride in the front seat of the vehicle used for transporting children.

I observed a nine month old asleep during this inspection. The infant was observed with a bib around his neck. A toy was also observed near the sleeping infant.

I observed two children sleeping behind a book shelf in the Pre-K classroom. The caregiver was not able to supervise all napping children in the room.
The medication log was reviewed and showed the following information was missing on several entries: -Dosage given -Date and time given -Employee's full name
Four classrooms were observed during nap time. All four rooms did not have adequate lighting. Note: This was corrected during inspection.
Feeding instructions were last updated on 09/01/2021 for two infants in care.
I observed a nine month old sleeping with a bottle.
