Kidz School Daycare Learning Center

A caregiver in the toddler classroom was unaware of the number of children present. The caregiver stated nine children were present, however ten children were observed in the classroom.

The medication authorization form was reviewed during this inspection and showed a staff member administered medication to one child in care. The dosage given was not documented.

During this inspection I observed unlabeled bottles and cups in the toddler and pre-k classrooms. Note: This was corrected during inspection.

An immunization record is needed for one child in care.

During this investigation it was determined the director allowed a staff member to transport children before completing transportation training.

During this investigation it was determined three children ages 4, 4, and 11 years old were not using safety belts while being transported.

During this investigation it was determined a driver who was transporting children did not complete transportation safety training.

Based on the information obtained, it was determined an eleven year old was allowed to ride in the front seat of the vehicle used for transporting children.

One child enrolled is in need of an updated immunization record.

During this inspection I observed one child asleep with a training cup. Note: This was corrected during inspection.

Documentation of all safety drills conducted for the year 2022 were not available for review during this inspection.

I observed four infants age 6m, 9m, 13m, and 13m sitting at an eight seat table. The safety straps were not utilized.

Five caregivers needed documentation showing how they meet minimum education qualifications.

Records were evaluated for ten children in care and the following was determined: One child needed a health statement. One child needed emergency care information. Three children needed emergency contact information.

I observed a broken swing on the outdoor playground. Note: This was corrected during inspection as the director removed the equipment.

The operation had not notified the parents of the safe sleep deficiency. Note: The operation completed Form 2970 Notification of Safe Sleeping Deficiency during inspection.

I observed a nine month old asleep during this inspection. The infant was observed with a bib around his neck. A toy was also observed near the sleeping infant.

The last certification was completed 06/01/2020.

I observed two children sleeping behind a book shelf in the Pre-K classroom. The caregiver was not able to supervise all napping children in the room.

The medication log was reviewed and showed the following information was missing on several entries: -Dosage given -Date and time given -Employee's full name

Four classrooms were observed during nap time. All four rooms did not have adequate lighting. Note: This was corrected during inspection.

Feeding instructions were last updated on 09/01/2021 for two infants in care.

I observed a nine month old sleeping with a bottle.

The last fire inspection was last conducted 06/26/2020.

One personnel record reviewed did not have current CPR/First Aid training.

Children did not wash their hands before eating food.

One personnel record evaluated needed preservice training.
