The center director has provided training to staff when the center has been assesed an administrative penalty within the previous 2 years.

A caregiver did not have a child wash their hands after diaper changing.

Play equipment was observed on the outdoor play area with rubber pieces for loose fill material that is installed over concrete.

3 out of 8 children's records evaluated did not have the name and address of the emergency-care facility.

The following children's records evaluated were missing the information listed below: 1 out of 8 children's records evaluated did not have an field trip authorization. 1 out of 8 children's records evaluated did not have an water activities authorization. 1 out of 8 children's records evaluated did not have an address for the emergency contact.

1 out of 8 children's records evaluated did not have a immunization record.

Loose-fill material was observed installed over concrete.

2 pedals on the Merry-Go-Cycle were missing.

6 out of 8 children's records evaluated did not have a special care needs statement.

During the follow up inspection another child was observed sitting in a high chair. It was stated by a caregiver that the child was in the high chair because they were running around and taking a long time to eat.

During the inspection children were observed climbing on chairs and tables. One child ran down the hall way into another classroom.

Caregivers were observed grabbing children by the arm to get them to sit down during the inspection.

A child was observed during snap time using a tablet. Note: This was discussed with the provider during the inspecdtion that screen time cannot be used during meals, snacks or naptime. It was suggested that tablets only be used during planned activities.

During the inspection a child was observed sitting in a high chair while playing on a tablet. When a caregiver was asked why the child was in the highchair it was stated because the child was special needs and would not stop running around. Note: It was discussed that no child can be confined to any type of restrictive device as a for of discipline.

During the inspection a caregiver was observed in the infant classroom with direct access to children. The operation has not received an elgiblity notification from CBCU stating the caregiver was allowed to be present. Note: The caregiver left the operation during the inspection and another qualified caregiver was moved nto the infant room.

Another child was observed sitting in a high chair and the safety straps were not secured. Note: The provider secured the straps during the inspection.

Children were observed climbing on top of chairs and running around the classroom during the inspection. A child also walked out of the classroom and the caregiver closed the classroom door. When opening the classroom door the child was observed walking back from to the classroom with no supervision.

Based upon the information obtained, there is sufficient evidence to support caregivers are using cell phones while caring for children.

Based upon the information obtained, there is sufficient evidence to support that the incident report for a child who was injured was not shared with the parent.

Based upon the information obtained, there is sufficient evidence to support a staff person from the operation did not sign the incident report for a child that was injured while in care.

Based on the information obtained, there is sufficient evidence to support parents are not informed of less serious injuries at pick up time.

Caregiver was observed not washing toddler hands after changing diaper. This was corrected during inspection when the caregiver was observed washing the child's hands.

A caregiver with conditions on background was observed caring for a group of children alone in the classroom. This was corrected during inspection when another caregiver arrived to work in the classroom with the caregiver.

Sippy cups iin the toddler room were observed not properly labeled with the child's first name and first initial of last name. This was corrected during inspection when the caregiver was observed properly labeling the sippy cups.

The safety alarm in the transportation vehicle was observed not properly working during the inspection.

The playground was observed having hazards( trash) throughout the playground accessible to children as they play. This was corrected during inspection when the caregiver was observed removiing the trash from the playground.

A 22 month old child was observed having screen time. This was corrected during the inspection when the caregiver was observed turning off the television and giving the child another activity to do.

An employee no longer associated with the operation had not been inactivated within the required time frame.

The caregiver did not provide activities for the children in care. The children were utilizing the stack of sleeping equipment as a form of play.

The caregiver in charge of a group of children did not intervene when the children were utilizing a stack of sleeping equipment as a form of an activity. The caregiver did not intervene when the children fell from the jumping off the stack of sleeping equipment, subsequently obtaining an injury. The caregiver did not intervene appropriately when the children fell off the stack of sleeping equipment. The children walked towards the caregiver.

Based on the investigation and information obtained the operation is not in compliance. Note: The caregiver is no longer employed at the operation.

A caregiver dropped off a child late at their school and did not follow the school's safety protocols to escort the child inside the building when there are no school staff in the front of the school to greet and open doors for the students.
