Based on the training certificates reviewed it was discovered that some of the staff did not complete required annual trainings within the allowed subsequent 12-month period. These trainings include Psychotropic Medication for Children in Texas Foster Care Training and Preventing and Recognizing Sexual Abuse and Victimization of Youth in Foster Care Training.

When reviewing the EBI policy, it stated that a personal restraint may not exceed one hour.

A child in care sustained an injury that required treatment by a medical professional and the operation failed to report the incident to statewide intake.

A bottle of what appears to be laundry detergent or fabric softener and an aerosol can of disinfectant/air freshener was observed to be in a resident's bedroom and accessible to all children in care.

The operation's staff did not follow their written policies and procedures regarding hazardous chemicals use and storage when two cleaning/laundry products were observed to be unsecured and easily accessible to children in care.

All posted evacuation diagrams were modified and included a designated place written however did not match with the area distinctly marked with a star in the emergency evacuation diagram. The diagrams are inconsistent and inaccurate.

Licensing was not notified of children's unauthorized absence until nearly 12hours after the children left the operation without permission.

Three out of three youth interviewed stated a caregiver makes smart alleck statements toward them.

All posted evacuation diagrams should have a designated place written or outlined in the emergency evacuation diagram.

On June 21 in the girls house in the refrigerator there was a gallon of milk was observed with an expiration date of May 18.

2 of 2 binders containing medication records were observed on a desk in the common areas.

2 of 7 children's file reviewed were observed without a signed copy of child's rights.

All nutrients menus were observed without food substitutions documented on the menus.

4 of 4 bathrooms accessible to children were observed without soap near the handwashing sink. 1 of 4 bathrooms accessible to children were observed without toliet paper.

All posted evacuation diagrams were observed and a supervisor confirmed that there was no idenified for sheltering inside.

3 of 3 child discharge files were observed without documentation confirming when the children were informed of thier discharge or tranfer.

An electrical outlet cover was observed in the boys house in the bedroom to the right to be missing on an outlet actively being used by children in care to charge electronics.

2 serious incident reports were observed with inaccurate dates of birth of children in care.

A child didn't return home from school at the regularly scheduled time which is approximately 3:46-4:00pm. The unauthorized absence was not reported until 1:41am the following morning.

The operation failed to submit quarterly EBI data for the 4th quarter on time.

Report was not able to be given at time of inspection.

On 12/5/22 a child received medical treatment at Brazos Urgent Care. This situation was not reported to the hotline as required.

5 children admitted that a staff member uses vulgar language in front of them.

It was noted that Ms. Sweed's EBI training expired on 05/12/22.

It was found during the course of the investigation that staff got into a verbal altercation with a child in care's visitor. In addition, it was also found that a staff sometimes yell.

The operation did not have an annual summary log readily available for review.

Two residents interviewed stated a staff member curses at them. One staff member interviewed stated it has been reported to her that a staff member curses at the residents.

Two children in care expressed that a staff grabbed a child in care around the neck and threw him onto the couch to stop them from potentially fighting.

Direct Care Staff most recent Trauma Informed Care Training was last taken on 10/19/20.

A child received two doses of medication that was not prescribed to them.

It was found that the organizational chart was not updated and was missing names of persons under positions listed.
