The director failed to operate in compliance with minimum standards and failed to ensure all employees comply with the minimum standards.

The operation served an identified food to a child with food allergies, resulting in the use of an epi-pen and ambulance being dispatched.

2 out of 2 infants not yet on table food requiring written feeding instructions were outdated.

The director needs to complete 2 hour transportation training and keep the training certificate in the staff file and made available to CCR for review.

4 out of 4 infants is missing the daily report. Daily reports are made on Brightwheel, however the caregiver was not able to access the daily report for review by CCR.

Outdoor play equipment on the children's playground has two screws protrudiung from the structure.

2 active ant beds are present on the playground near the active play equipment.

Part of the fence is laying on the ground and needs to be stood back up or replaced.

The children have not been outside since June 2024 for outdoor physical activity.

4 out of 4 infant bottles and cups are missing the required name label.

The hand washing sink in the diaper-changing area in the infant room is out of service.

The toddler room floor has multiple areas of ripped/ torn vinyl flooring that needs repair or replacement.

A fire inspection has not been completed since October 27,2022..

One feeding instructions sheet for an infant on bottle food was last signed by the parent in August 2022.

A large spiny cactus has grown up from the neighbors yard along the playground fence, made accessible to children during play.

Multiple active ant beds in the playground were observed.

Two tricycles, one plastic see-saw, and one child chair located in the playground, were damaged with sharp edges. Note: this was corrected at time of inspection when the director removed it.

The use zone for the exit of the slide is measured at 3 1/2 ft from the barrier.

The loose fill is measuring at 3 inches out of the 6 inches.
