A 3 mo old infant was observed in a baby bounce. When asked how long the infant was in the bouncer, the caregiver stated one hour.

Records were reviewed for 5 children currently enrolled, 2 children did not have an address listed for the emergency contact, 2 did not have an emergency contact listed, one did not have doctor's information listed and one did not have a special needs statement completed.
Of the 8 children transported, 6 did not have completed emergency contacts.
A slide with a designested play surface approximately 4ft in height did not have surfacing.
Upon arrival, a 3 yr old was observed sitting in a high chair. The caregiver indicated the child was placed there becuase the child was "extermely active". Another caregiver indicated children are placed in the high chair for time out for a period of 5-10 minutes.
Based on the information obtained during the investigation, there is sufficient evidence provided to support the allegation of the director yelling at children.
