The cots were not labeled during the inspection. Note: This was corrected during the inspection when the asst. caregiver labeled each cot with each child name.

The director was missing hours in management techniques, leadership, or staff supervision

Two out of five children files was missing health statements.

One out of five children files were missing immunization records.

Three out of three files was missing the following: documentation showing the dates of the first and last day on the job, Documentation showing how the caregiver meets the minimum age and education qualifications and a copy of a photo identification.
