Notification of field trip was not posted in a prominent place were parents and others may view it. This notice must remain posted until all children on the field trip have returned to the daycare.
Based on the investigation conducted by DFPS and the follow up conducted by Licensing representative nothing was posted to notify parent of the field trip to director's home and the operation did not receive any signed permission from parents who went on this field trip.

During the follow up inspection 5 children in the infant room and 6 children in the pre-kindergarten room were not signed in. The sign in sheets had not been printed for the new month. This was corrected during the inspection when each child was signed in.

A caregiver with provisional eligibility was observed alone with 11 children reading them a story. The provisions state the caregiver must never be left alone with children. Note: I discussed all provisions of eligibility must be followed with the director during the inspection.
I observed an infant resting in a crib. The infant had a pacifier with an attachment posing a choking hazard. This was discussed during the inspection that these items pose a risk to infants and should not be used.

A child walked out of the classroom, down the hall way to the front of the center and the caregiver could not see where the child was.

Diaper changing mats in the infant room were covered with sheets. Note: The provider removed the sheets during the inspection. It was discussed that sheets prevent the diaper changing mat from being non absorbent and easy to clean.
A caregiver was observed in the infant room during the inspection. The caregivers background check is still pending and the operation has not obtained an eliglibity notification from CBCU.
Sign in and out logs in each classroom observed during the walk through were not updated. In the infant room, the sign in sheet was blank for the last 4 days. Children were not signed in and one caregiver did not know where the sign in sheet was for the three year old classroom.
Electric Fans placed on top of shelves and tables in the infant and toddler rooms were unstable and posed a risk to children. Note: The provider removed the fans during the inspection.
