Upon arrival, an 11 month old was observed to be sleep with several toys in the cdrib. NOTE: The child was asleep for the duration of the inspection. The toys were observed in the crib for 15 mintues before being removed by the caregiver.

The operation has not received their annual fire inspection. Last dated inspection was 12/26/2023.

Multiple shelves were observed to be used to create a separate area for your youngest infants. The shelves blocked to only path to the door.

A caregiver was observed changing 2 diapers. The caregiver wore a glove one one hand. The caregiver did not remove the glove after either diaper change and did not wash their hands in between the diaper changes.

6 infants between 8mo-11mo were observed with 1 caregiver. The caregiver was over ratio by 2.

A 2yr old was observed to be sleeping with a training cup.

A group of 2yrs old was observed to be watching Frozen during nap time.

Based on DFPS investigation there is enough evidence to validate that a six-year-old child was placed in a highchair as a form of discipline.

Based on DFPS investigation there is enough evidence to validate that a caregiver did not intervene to keep one child from hitting another child.

Based on DFPS investigation there is enough evidence to validate that a caregiver humiliated a child when a six-year-old child was not treated with respect due to being placed in a highchair for discipline.

All Licensed childcare centers must annually report to CCR the number of employees who left employment the previous year by January 15. The deadline for entering the number of employees who left employment in 2023 is 1/15/2024. If you have not already you must log in to your childcare regulation account and report the number of employee that left your employment in 2023

Notification of field trip was not posted in a prominent place were parents and others may view it. This notice must remain posted until all children on the field trip have returned to the daycare.

Based on the investigation conducted by DFPS and the follow up conducted by Licensing representative nothing was posted to notify parent of the field trip to director's home and the operation did not receive any signed permission from parents who went on this field trip.

There was no Emergency medical authorization on file for one out of four files checked.

Notarized affidavit was missing on file for two out of four files checked.

Date of employment was missing on file for one out of four staff files checked.

Proof of education was missing on file for one out of four staff files checked.

Three out of four staff files checked did not have any notarized affidavit. Two were corrected during inspection.

Three children seated at the highchair table did not have their safety straps on. This was corrected when director replaced all the safety straps for proper use and convinience.

Emergency medical authorization information was missing on file for one out of five files checked.

Director was missing first aid and Cpr training. She said she has it but could not find at the moment.

One child did not wash his hands after toileting. This was corrected when caregiver washed his hands.

Date of admission was missing on one out of five files checked. This was corrected during inspection.

Release information was missing on one out of five files checked.

A copy of immunization for one child was not on record.

I did not observe any transportation training for the director of this operation.

Immunization record was missing on file for one out of six records checked.

First and CPR training was missing on file for one out of four staff files checked.

I did not observe any current liability insurance.

There is no acknowledgement that one caregiver received orientation or preservice training.

I did not observe any current annual sanitation inspection. The one on file had expired since 8/13/2022.

One caregiver had two out of twenty four hours of annually training. This caregiver was hired 6/28/2021. Another caregiver hired 3/1/21 also did not have any training hours for the period of 3/1/2021 to 3/1/2022.

Physician information and Emergency medical authorization information was missing on file for one out of six records checked.

One caregiver did not have the acknowledgement that she received any center policies on file.

One caregiver was missing photo identification on file.

There was no documentation of monthly fire drill for the month of November, the last fire drill documented was in October and we are now in December.

Four infant feeding sheet were not up dated.

Health statement was missing on file for one out of six records checked.

One of the classrooms was not observed to have adequate lighting during naptime. The number of children present could not be seen. NOTE: Corrected at inspection. A light in the hallway was turned on to provide more lighting in the classroom.

Based on the information obtained during the investigation, there is sufficient evidence to support the allegation of a child being dragged by the arm by a caregiver.

As of 5/25/2021, the provider still has not submitted proof of one caregivers training hours being completed.

During the follow up inspection documentation of current Transportation training for the director was still not available.

During the follow up inspection 5 children in the infant room and 6 children in the pre-kindergarten room were not signed in. The sign in sheets had not been printed for the new month. This was corrected during the inspection when each child was signed in.

1 out of 4 staff records evaluated did not contain documentation showing CPR/First Aid Training.

A caregiver with provisional eligibility was observed alone with 11 children reading them a story. The provisions state the caregiver must never be left alone with children. Note: I discussed all provisions of eligibility must be followed with the director during the inspection.

I observed an infant resting in a crib. The infant had a pacifier with an attachment posing a choking hazard. This was discussed during the inspection that these items pose a risk to infants and should not be used.

Admission information was incomplete for 9 out of 10 children's records evaluated. The following information was missing or incomplete: Emergency Contact Information Authorization for Emergency Medical Treatment Health Statements Immunization Records Acknowledgment of Centers Policies

There is a yellow slide in the outdoor play area that does not contain loose fill material. 6 inches of loose fill material is required for climbing equipment with a platform of 5 feet or less

A child walked out of the classroom, down the hall way to the front of the center and the caregiver could not see where the child was.

Three cribs in the infant room were not labeled with the child's name. Note- The provider added labels to the three cribs during the inspection.

8 out of 10 staff records evaluated did not contain documentation showing annual training hours have been completed.

The director does not have current transporation training.

Diaper changing mats in the infant room were covered with sheets. Note: The provider removed the sheets during the inspection. It was discussed that sheets prevent the diaper changing mat from being non absorbent and easy to clean.

A caregiver was observed in the infant room during the inspection. The caregivers background check is still pending and the operation has not obtained an eliglibity notification from CBCU.

Sign in and out logs in each classroom observed during the walk through were not updated. In the infant room, the sign in sheet was blank for the last 4 days. Children were not signed in and one caregiver did not know where the sign in sheet was for the three year old classroom.

Electric Fans placed on top of shelves and tables in the infant and toddler rooms were unstable and posed a risk to children. Note: The provider removed the fans during the inspection.
