Two children were observed eating breakfast while the TV was on. This was addressed during the inspection and the director turned off the TV.

Two children with a medically diagnosed allergy did not have a food allergy emergency plan in there files.

Two school age children were observed playing with personal tablets in the main room with a mixed age group. The activities on devices were not related to a planned activity and the caregiver was not aware of what the children were doing on their devices and the caregiver stated the parents wanted them to be on there tablets because it is summertime. The other children were also watching TV that did not relate to the curriculum and planned activities. This was corrected during the inspection when the director turned off the TV and put the children's tablets away.

The maintenance procedure checklist was not available for review.

Two out of four children did not have a date of admission .

The playground area had weeds greater than eighteen inches.

The caregiver did not have access to accurate information showing each child's age.

Two out of four children did not have health statements available for review.

A large unmounted fan was accessible to children. This was corrected during the inspection when the director turned off the fan and removed it from the area.

The playground had a wooden board with a nail sticking out.

Four caregivers did not have the handling, storing, and disposing materials training available for review.

The use zones on the play structure are not a minimum of six feet in all directions.

The slide on the playground structure does not meet the six feet use zone requirements in all directions.

The playground area does not have a second exit.

Infants were combined with children more than 18 months older than them. The youngest child was twelve months old and the oldest child was six years of age.

Two caregivers did not have the required 24 hours of annual training. Both caregivers were missing training in the required topics of Handling, Storing, and Disposing of Hazardous Materials, Administering Medications, Preventing and Responding to Emergencies due to Food Allergies, Emergency Preparedness, Preventing the Spread of Communicable Diseases, and Building and Premises Safety. Also missing training in the required topics of SIDS/Shaken Baby.

The director did not have annual training hours in the required topics of Handling, Storing, and Disposing of Hazardous Materials, Administering Medication, and Preventing and Responding to Emergencies due to Food Allergies, Emergency Preparedness, Preventing and controlling spread of communicable diseases and Building and Physical premises safety.

There were two baby walkers observed in the nap area. Director removed both baby walkers.

Current annual inspection was not available during inspection.

Current first aid and CPR were missing on two out of four staff files checked.
