Based on the information obtained during the DFPS investigation, the driver who transported a group of children, failed to ensure all of the children who loaded the vehicle were properly unloaded. Note: The driver deviated from their normal route to pick-up/drop-off another group of children, as a result, the driver was unaware a child was sleeping on the seat of the vehicle and had not been dropped off at school until the child's elementary school inquired about the child's absence.

Documentation was not available for review to support a caregiver's high school graduation from outside of the United States. Out of the five records for staff evaluated during the inspection, one did not complete their required training by 05/20/2023 (missing 14 clock hours); three newly hired caregivers w/o previous experience, did not completed 8 hours of training before being in the classroom. Hire dates 08/14/2023, 08/21/2023 and 08/23/2023; one caregiver who supervises children under 24 months did not have verification they completed Sids/Shaken Baby and Brain Development Training. The director had completed 30 clock hours of their annual training on/before 07/29/2023.
Four infants under 12-months old were observed with blankets in their crib. Note: Three of the infants were sleep and covered with the blanket and the 4th was being put to sleep with the blanket observed in the corner of their crib.
