A 5 month old infant was observed to be sleep in a baby bouncer with a blanket over them

The Children's Products Safety certification was last completed 10/2018

A boppy pillow recalled 9/23/21 was observed to be in the infant room and in use by an infant in care.

The fire inspection available for review was last conducted on 08/22/2022.

Documentation was not available for review to support a caregiver's high school graduation from outside of the United States. Out of the five records for staff evaluated during the inspection, one did not complete their required training by 05/20/2023 (missing 14 clock hours); three newly hired caregivers w/o previous experience, did not completed 8 hours of training before being in the classroom. Hire dates 08/14/2023, 08/21/2023 and 08/23/2023; one caregiver who supervises children under 24 months did not have verification they completed Sids/Shaken Baby and Brain Development Training. The director had completed 30 clock hours of their annual training on/before 07/29/2023.

Four infants under 12-months old were observed with blankets in their crib. Note: Three of the infants were sleep and covered with the blanket and the 4th was being put to sleep with the blanket observed in the corner of their crib.

The current gas inspection available for review was conducted 03/16/2020

Based on the information obtained, it is determined that there is evidence to support that the the operation failed to report the incident placing a child at risk.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

The staff training records were not available for review during the inspection.

The facility owes a total of $26 for background checks submitted. Note: A $16 invoice was sent on 06/02/2021 and another invoice for $10 was sent on 09/02/2021.
