A three month old infant was asleep in a bouncer.

In the caterpillar one classroom three children were not strap in the high chair. In the infant classroom two children were not strap in the bouncer and one child in the high chair. Corrected: When caregiver strap child in the high chairs and removed children out of the bouncers.

One child did not have a signed form stating they received the operational policies.

One child did not have the required information to have their immunization exempt.

The operation updated the information on the form 7250 for the month of July. The director and I spoke about writing staff names who were present doing the observation.

The operation did not notify CCR inspector of the change in toddler classroom when added a divider in the classroom to make the classroom two different classrooms.

They were 25 children in the gym youngest child was 14 months old and the oldest child was 5 years old. They were three caregivers in the gym caring for the children. It was a difference of 18 months from the youngest child to the oldest.

The operation reduced the child/Caregiver ratio for a infant classroom specified age group 12-17 months old. When a caregiver left one caregiver with eight infants which would be three over the ratio. Corrected: When the director had someone go into the classroom with the other caregiver.

In a crib they was a pacifier with a stuffed animal attached. In another crib they was a 10 month old infant with a blanket.

In the infant classroom two cribs had blankets covering the front of the crib and the caregiver was unable to see the infants.

This was corrected when the infant was found to be awake and the caregiver went on the other side of the wall and removed the infant from the crib.

One caregiver hired 1/29/2024 caring for children 24 months and younger did not have preventing shaken baby syndrome and abusive head trauma, early childhood brain development and SIDS.

After the caregiver changed a child's diaper, she place the child on the ground and did not was her hands. Corrected: When the caregiver went back and washed the child's hands before she changed another child.

The operation did not use play equipment as stated in the manufacturer's instructions when they did not acre the climber down or have it outside on the playground. The director is not ensuring that classrooms are maintaining the correct ratio and that infants are not mixed in with school age children. The director is not ensuring that caregivers are follow minimum standards.

The director is missing 11 1/2 hours of annual training.

One caregiver is missing 14 hours of annual training.

Infants were in buggy ride for over 30 minutes, the infants did not have the opportunity to have floor time. They were some infants that were fuss during this buggy ride.

One children is missing a emergency contact information and 3 children were missing an address for their emergency contact.

The two year old classroom and school age children were outside on the playground at the same time, the groups were mixed together for approximately 15 minutes.

In the infant and two year old classroom did not have lids on the trash can where garbage is accessible to children.
