The operation only conducted 2 shelter drills and lock down drills within the last year.

A caregiver was observed on a laptop doing other activities that were not directly related to the caring for children in the class.

Documentation of the full 30 hours of annual training for the director has not been completed.

1 out of 8 children's files evaluated did not contain authorization for emergency medical treatment and a special needs statement.

A gas leak inspection has not been completed January 6th, 2021.

There were no children signed in for the pre-k classroom during the inspection. Note: This was corrected during the inspection.
