First aid licenses was observed exipred for 1 caregiver 3 other caregivers cpr/first aid licenses could not be observed.

Caregivers folders were observed to be missing several documents and trainings.

4 of the 5 children records observed did not have a completely filled out admission form, immunizations and health statement information.

All four caregivers folders were observed to be missing several documents.

4 baby walkers with wheels were observed with infants in them.

Based on the attendance log from 09/28/2021-09/30/2021, a total of 6 infants were signed in, 0-12 months of age with one caregiver.

Based on information obtained throughout the investigation, it was found that one operation staff may supervise two classrooms during pick-up time. It was found that one caregiver stands in the doorway to supervise 2 age groups.

Director fail to ensure a caregivers left in charge of groups of children without the required pre-service training, SIDS, Infant, and CPR/ First Aid. I observed 1 of the 4 caregivers to have CPR/ first aid licensing and preservice training. 1 of the 4 caregivers folders could not be observed.

3 caregivers did not have a valid CPR/First aid license to observe.

2 caregivers with hire dates of 06/02/2021 and 07/13/2021 did not have preservice training and orientation.

It was discovered through CBCU rep a caregiver was working at the operation without a cleared background check. A phone call was made to Director to verify and she stated Caregiver worked/volunteered for 1 week in the school-age classroom.
