11 month old was observed to be asleep in a bouncer for approximately 5 minutes, caregivers pointed towards the child and walked passed them several times before removing the child and placing them in a crib.

Director and caregiver that transports children did not have the required transportation training.

One caregiver in the infant room did not have the required SIDS training.

There were no infant daily report sheets available to review.

I observed children under the age of 2 watching tv. NOTE: This was corrected during the inspection by turning the tv off.

I observed children not washing their hands before snack time.

An 3m, 7m, 10m, and 11m was observed with a blanket in their crib. The 11m old was also observed drinking a bottle in their crib.

Two classrooms were observed not having a number chart to match the numbers labeled on the cots. NOTE: This was corrected during the inspection with a chart made to match the numbers on the cots.

One caregiver was observed with 9 children and SAG 0-11m.

There were 9 children in the infant room, but 8 were signed in. There were 13 children in the 2 year old room and none of the children were signed in. There were 18 children in the 2-4 year old room, but only 13 were signed in. Note: This was corrected by the caregivers during the inspection.
