The carbon monoxide detector was stored in a cabinet and has not been tested on a monthly basis since the permit was issued. Note: The provider tested the carbon monoxide detector during the inspection.

The providers permit was not posted in the home.

Clorox cleaner was observed under the sink in an unlocked cabinet in the restroom used by children and was accessible to children.

The provider has not tested the smoke detector on a monthly basis since the permit was issued. Note: The smoke detector was tested during the inspection.
