Diaper changes were observed. 2 children were not made to wash their hands following a diaper change.

Upon arrival of a caregiver, 2 children were taken into a classroom and an infant was left in a hall way.While in the room, the 2 children were observed to be left unspervised multiple times while the caregiver assited the director in looking for training certifcates on their phone and when going to wash their hands following a diaper change. The infant was left in the hall multiple times with CCR.

The double doors on the side of the van used could not be opened from the outside. Open door could not be opened, the other door had to be opened from the inside.

During the follow up inspection, the director and one other caregiver both have pending background checks. The operation has not received the eligibility notification from CBCU stating they can be present. Note: The director and the caregiver left the operation during the inspection.
The playground contained rusted bolts coming out from the wood pieces, trash and debris including empty cans and bottles, exposed wires and dog feces. An ant bed was also observed on the playground and there are two toilet seats in the restroom that are broken.
During the inspection it was observed that the background check for the director was in an inactive status since May 31st, 2023. Note: The director submitted the background check during the inspection.
Floor tiles in the restrooms were cracked and pose a tripping hazard to children.
During the inspection, the hand washing sink was not equipped with soap.

A time sheet or log was not available for the director. According to the owner, the director is not required to sign in or out.
Napping cot were observed to be arranged in a way that did not allow for the caregiver to supervise all children. One child was observed to be placed inside of a classroom, one child was placed in a hallway, and a third child was placed in between two rows of picnic style tables. From where the caregiver was standing, these children could not be seen.
One child's mat was observed to be placed in the entrance to a classroom, blocking anyone from being able to enter.
Personnel records were not available for review when requested. There was once caregiver present at the time of arrival. The caregiver stated they did not know anything about the files and that CCR would need to wait until the owner arrived.

The caregiver stated they were waiting until 9:30 for a language teacher to arrive. The caregiver was asked how long have the children been waiting. According to the caregiver, the children arrived around 8am and ate breakfast and have been waiting.

Two caregivers in charge of a group of children were not aware of the number of children in their care. The caregivers stated there were 17 children present. The caregivers were asked where the additional children were. The caregivers looked at each other and began talking. When asked again., one caregiver stated there were 12 children in care.
Caregivers in charge of a group of children 2y-7y were not aware of the ages of the children in their care. When asked for the ages of the children, the caregivers stated the oldest was 8yrs old.
Children in time out are required to face a wall for an unknown period of time. One caregiver stated they will have children sit for a period of 10-15 minutes.

Based on the information obtained during the investigation, the operation did not demonstrate good judgement when they failed to pick up a group of children from school. The parents were notified by their children two hours after dismissal that they had not been picked up by the operation. According to the owner, the operation's phone was unplugged because it was "ringing too much". The operation also failed to provide the school with an alterative contact for the operation.
A caregiver in charge of a group of children ages 18m-2yrs old was not aware of the ages of the children in their care. When asked, the caregiver stated the children were 4yrs old. When asked again to verify, the caregiver again stated all children were 4yrs old.
A caregiver was observed using the same gloves to change two children's diapers.
A caregiver was observed standing in the door supervising a group of children. The children were on the opposite side of a tall shelf and could not be seen. Another caregiver left a classroom of 4 children 18m-2yr old when Licensing entered the room and began asking questions. The caregiver did not return until approx. 3 minutes later when the director was notified that the caregiver left.
A caregiver was observed changing diapers of two children. The caregiver did not wash their hands between diaper changes or after.
A heavy self was used to block the only exit from the 3-4 yr old classroom. According to the caregiver, the shelf was placed to prevent a special needs child from running out of the room.
During the inspection, a special needs child was observed sitting in a chair near the careigver away of the other children. When Licensing enter the room the child got up from their seat and the caregiver was observed holding a child's hand and would not let go when the child attempted to play with toys. According to the caregiver, they were keeping the child next to them because the child was throwing toys.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022 revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022 as required. The director was observed watching two classrooms with children ranging from 3-12 yrs old. There were a total of 24 children present.
