The operation has a walker in the infant seahorse classroom which is prohibited equipment. An 8 month old infant was sitting in the walker and the walker wheels were unlocked.

The operation did not make a report when a child received medical treatment from an injury that occurred at the operation back in March 25, 2024.

Caregiver stated she did not use gloves when handling the child's bloody injury.

In the classroom on the three floor backpacks did not have a storage area for them, and they blocking an exit doorway.

The fence on the front area of the playground is not four feet it is 3 feet and 9 inches.

A child in the classroom on the three floor was blocking one of the entrances. Corrected: When the child was moved to another area of the classroom by the caregiver.

The operation is using space that is not license, this space is space that was added on to the building.

During the inspection I observed a child sleeping on a nap mat that was blocking the doorway. This was corrected when the caregiver moved the child and nap mat to another area in the room.

They was an employee in the building with a pending background check. The employee was taking supplies to classrooms.

The operation did not have in the operational policies on how parents could contact the local licensing office. Corrected: When the operation added the information in the operational policies.

The opeation did not have documentation for children being on the second floor.

The children's files are not updated with the new ownership and did not have the new enrollment date.

The six new employees did not have the updated affidavit form on file.
