The First Aid Kit lacked a Guide to First Aid and Emergency Care.

A child's hands were not washed after a diaper change. This was corrected at inspection.

Of the four children's records evaluated; the following information was missing: 1. Two lacked a shot record 2. Two lacked the address of the emergency contact 3. One lacked the name, address and phone number of an emergency contact other than the parent 4. Two lacked a health statement 5. One lacked the name and phone number of the school the child attends 6. Two lacked the required information regarding special needs 7. Two lacked the required information regarding water activities

This standard was evaluated and compliance was not met. The following information is missing from the operational policies: 1. The duration of outdoor play 2. The duration of physical activities 3. Safe sleep policy for children 12 months old and younger 4. Informing parents there is an open door policy for them to observe their child's activities

Children under the age of 4 were served whole grapes. Whole grapes are a choking hazard for children under 4 years old. This was corrected at inspection.

The child care home is not equipped with a carbon monoxide detector.

The primary caregiver has not completed a transportation safety training.

The required authorizations for emergency medical care for each child being transported is not in the vehicle used for transportation.

Two personnel records lacked a notarized affidavit.

There were uncovered outlets in the room used by the children.

One caregiver did not receive training in the following topics for the October 2022-October 2023 training year: emergency preparedness, administering medications, communicable diseases including immunizations, understanding building and physical premises safety and handling, storing and disposing of hazardous materials. The caregiver will need to receive make-up training hours that will not count towards the current training year.

The primary caregiver received 6 of the 30 clock hours of required training. The primary caregiver will need to receive 26 clock hours of make-up training. The make-up training hours will not count towards the current training year.

The home is not equipped with the proper size fire extinguisher.

There is no documentation of drills available for review.
