Two expired booster seats were in use by transported children.
Three school age children were observed playing games on the computer than included shooting, ramming of cars and fighting. This was corrected when the inappropriate games were deleted from the computer.
The fire extinguisher in one of the vehicles used for transportation needed to be recharged and would not have functioned in an emergency. This was corrected when a replacement extinguisher was placed in the van.
The two caregivers present in the school age classroom did not know the amount of children they were supervising.
A four month old n infant was in a bouncy seat that was placed inside of a crib with blanket over their legs..
In the infant classroom four infants were left unattended while the caregiver went next door to convesre with toddler's caregiver.

During the inspection, two classrooms did not have children signed in for the day on the tablets. Note: The provider signed all of the children in during the inspection.
During the inspection the outdoor play area contained large bolts projecting from the borders made of wooden logs. Trash and a tricycle with a cracked seat was observed in the outdoor play area. A caregivers purse in the infant room was left in reach of childrena and there are 2 broken cabinets that could fall and injure one of the children.
The gas leak inspection has not been conducted since November 3rd, 2019.

During the inspection, the outdoor play area did not contain adequate loose fill material. The playground with the yellow slide requires an additional 4 1/2 inches of loose fill and the play area with the blue metal slide requires an additional 3 inches.
Two tricycles were observed to have cracked baskets with large openings. One was observed to have a cracked seat. One was observed to have a broken handle bar. NOTE: Corrected at inspection. The baskets were removed from the tricycles and the tricycle with cracked seat and broken handle bar was removed and disposed of during the inspection
There were 5 climbing structures observed on the playground that did not have loose-fill surfacing filled to the appropriate depth. The ground and grass under the existing surfacing was observed to be exposed.
Three cribs in the infant room were not observed to have snug fitted sheets. One of the sheets was observed to be tied to the bars on the crib. One sheet was too tight and was observed bending the mattress in the crib. NOTE: Corrected at inspection. The correct size sheets were placed on all three cribs.
A 12 month infant was observed to be sleep with a bib attached to their neck. NOTE: Corrected at inspection. The bib as removed during the inspection
