During naptime in the toddler room, a christian video was playing while the children were falliing asleep on the mats.This was corrected when the video was turned off and music was playing.

A 3 month old infant was asleep in a restrictive device. This was corrected when the 3 month old was removed from the device and placed in a crib.

A person identified as a volunteer had unsupervised access to children in care in the infant room without having pediatric first aid training. There were 3 infants in care of the ages 3 months old and two 17 months old.

A person identified as a volunteer caregiver was counted in child/caregiver ratio while not having current CPR/First Aid training. This person was also found asleep while caring for 3 infants of the ages 3 months old and two 17 months old. The 3 month old infant was found asleep in a restrictive device. This person was identified as a volunteer by the director, but has been present at the operation consistently. There are no training records for this person.

In the infant room, two bottles of cleaning supples were on the low ledge of the window making them accessible enough to walking infants. This was corrected when the caregiver removed the cleaning supplies by placing them in a locked cabinet.

A person who was identified as a volunteer caregiver was found asleep in the infant room while one infant was asleep in a restrictive device and two infants sleep on mats. This volunteer was counted in ratio and left alone to care for infants. This was corrected when the caregiver was asked to wake up and remove the sleeping 3 month old infant from the restrictive device.

Requested files were not made immediately accessible for review. Files were requested at 11:30AM and were made available to review over the timespan of 5 hours. The director was assisting in and out of classrooms and assisting with lunch and nap time before providing licensing with the requested files. One caregiver file was never made available for review.

Based on documentation obtained for the investigation, it was found that one caregiver's training certificate shows inaccurate date information. This certificate counts a total of 30 clock hours completed on 7/8/23. There is a breakdown of clock hours for each training given on the certificate, but dates are not listed for each training as 30 clock hours cannot be completed in one day.

Based on documentation obtained during the investigation, records showing that a volunteer has been constantly working at the operation from April 12, 2022 without having started or completed 24 clock hours of annual training. This volunteer was counted in child/caregiver ratio and has cared for children unsupervised.

The operational policies or parent handbook does not state that parents may visit anytime during operating hours without requiring prior approval. In the parent handbook, it states that parents have to acquire prior approval before touring and visiting the school.

Two out of three caregivers were missing their educational qualifications on file.

One out of four children did not have current immunization records on file.

There was no record of lock down drills.
