I evaluated the enrollment records for 3 children. I observed that the records were missing the following information: 1. Two records did not an admission date. 2. One record did not have an emergency contact person listed. 3. Two records did not have names and phone numbers for persons to whom the child can be released. 4. One record did not have parent permission for emergency medical care. 5. Two records did not have any information indicating whether the child has special needs. 6. One child did not have documentation of a preschool heatlh statement.
