During the walk through, it was observed there inside the toddler room on the wall by the entrance door, the light switch is broken and had chip paint. Photo was taken at inspection.

During the walk through, it was observed there were dust and debris with cups and containers inside the toddlers classroom sink, open accessible to children. Outdoor playground was observed, there were debris of trash at the corner of the gate, rugs, and toys. Photos were taken during the walk through.

During inspection it was observed two infant bottles used was not label and placed inside of the refrigerator .

I observed a bolt protruding from the outdoor playground fencing.

I reviewed four children files. Two of the files did not have a health statement or a statement from the parent stating the child is healthy for school. One file did not have emergency contact information. One of the files did not have a signed acknowledgment of receiving the operational policies.
