Dog feces were observed on the play area were the children play and a white wooden board was observed broken with a nail sticking out of it. During the inspection the primary caregiver did remove the feces and threw it in the trashcan and nailed the wooden beam back into the fence.

During the inspection a space heater was turned on placed on the floor in the kitchen in reach of children. A kitchen knife was observed sitting on the table in reach of children. Note: These were corrected during the inspection when the provider put them away.
