The caregivers in the Discovery Preschool room was unaware of the number of children responsible for. A caregiver stated 15 children were in the room, however 16 were observed in the classroom.

The Emergency Preparedness Plan did not include How the operation will reunify the children with their parents as the evacuation, relocation, or sheltering/lock-down is lifted.

The Emergency Preparedness Plan was reviewed during inspection and the following is needed: Instructions for evacuating and relocating children who are under 24 months of age, who have limited mobility, or who otherwise may need assistance in an emergency. The staff responsibility in a sheltering/lock-down emergency for the orderly movement of children to a designated location within the center where children should gather. The alternate shelter away information provided to parents needed updating.

Three staff members need training in Understanding early childhood brain development.

Updated immunization records is needed for three children enrolled.

During today's inspection I observed a group of children napping in the Discovery Preschool class ranging from ages 14 months old-3 years old. The oldest child was more than 18 months older than the youngest child.

During today's inspection I observed children laying on numbered nap cots in the Pre-K room. The number/child assignment chart posted was not followed. A number/child assignment list was not available in the Preschool room. Note: This was corrected during inspection.

Bottles in the Infant Room were missing a label. Note: The provider labeled the bottles during the inspection.

A dead bug was observed on the floor by a shelf in the classroom that has not been disposed of. Note: The opreation disposed of the dead bug during the inspection.

Training certificates were missing the following information: - Trainers Qualifications - Participants Name - Clock Hours

During the follow up, there were still two screws missing on the climbing equipment in the DPS classroom causing the equipment to be unstable. There are still uneven cracks on the outdoor play area that also pose a tripping hazard for children who are playing on or near the surface.

Mangos were served with lunch instead of pears as listed on the daily menu. Note: Provider corrected this at inspection by documenting the substitution on the provider's app.

A child was observed in a classroom alone with no caregiver standing at the sink. Note: Sueprvision is criticial to the safety and well being of children in care. Caregivers must have visual and audio awareness to maintain an adequate level of supervision.

During the inspection brown stains were observed on the ceiling tiles in the classroom.

Children did not have their hands washed after coming inside from their outdoor activities during the follow up inspection.

A child in the two year old classroom was grabbed by the upper arm and sat down in their chair. Note: It was discussed with the provider the proper way to direct children to sit down by holding and picking them up from underneath their arms.

Documentation of current liability insurance was not available for me to review during the inspection.

During the inspection a child was observed sleeping in a crib with a pacifier that contained an attachment piece to their clothing. Note: This was addressed during the inspection by explaining to the operation that pacifiers with attachments are a strangulation and choking hazard.

During the inspection, a black cap was missing from a play structure on the younger children's playground.

During the inspection the Pre-Kindergarten classroom did not have 3 children signed in. Note: This was corrected during the inspection when the caregiver signed the 3 children in.

A caregiver does not have current CPR and First Aid training. The caregivers training expired on April 26th, 2023.

In the preschool discovery and toddler classrooms, the naptime cots were not labeled. Note: Corrected when the cots were labeled with the children's names.

There was no documentation showing a severe weather drill was conducted this year.

Based on information obtained, there was sufficient evidence to support rodents were observed at the operation.

Two caregivers with pending background checks were present at the operation and had access to children in care.

Based on information obtained, there was sufficient evidence to support children were not adequately supervised as three toddlers walked out of the exit door of their classroom and were on the playground unsupervised.

Several nap cots were observed to be placed behind shelves. From the caregivers position in the room, the children could not be seen.

In the pre-k bathroom, puddles of water was observed on the floor.

The director did not have current training in SIDS, shaken baby and early brain development.

Two toddler classes combined and the caregivers in charge were not aware of the ages of the children they combined with. They caregivers were asked several times of the ages of the children they combined with and could not provide an anwser. When a third caregiver walked into the room, they informed Licensing the children belonged to that caregiver and knew the children's ages.

A child was observed sleeping in front of an exit door.

Two caregivers with a hire date of 6/1/21 and 8/16/21 did not have documentation of completed orieintation training.

Training certificates reviewed for four staff did not show that the staff had current training in Early Brain Development.

Records were reviewed for 5 staff including the director. First-aid training for one caregiver was non-pediatric and two employees did not have current first aid training.

Several training certificates observed in the director's file and an infant caregiver's file did not have the trainers qualifications listed.

CPR training was reviewed for 5 staff including the director. Training for the director and two additional caregivers was received online with no hands-on training.

A child was observed laying on a stack of cots, the child then stood on the cots several times and attempted to climb on a cubby above. During naptime several children were observed running around the classroom.

An infant who is not yet on table food did not have updated feeding instructions. NOTE: The instructions were last updated 7/3/21

Several times throughout the inspection, parents were observed being stopped at the door, it was learned that parents are not being allowing inside of the building due to the operations policy.

Based on the information obtained during the investigation there is sufficient information to support the allegation regarding pest. Dead pest and rodent droppings was observed in several areas of the operation, Pest control services made several recommendations for building maintenance that was not followed.

Based on the information obtained during the investigation, there is sufficient information available to support parents were not notified of a situation that placed children at risk.

Operation failed to notify licensing of an intrusion at the daycare center and records being stolen.

Based on the information obtained the operation will be cited for two children exiting the classroom door and going onto the playground unsupervised. Note: Operation is in compliant and I spoke with the operation regarding supervision and the importance of physical proximity, audio and visual awareness. Caregivers have also taken refresher courses on supervision.
