There was a 3 year old child in the infant classroom when the inspector arrived at the operation. The child was more than 18 months older than the 6 month old in the classroom. NOTE: The child was returned to her pre-k classroom immediately.
A child was observed sleeping in a crib on their tummy and was covered by a blanket. The child can roll over. The blanket was removed from the crib during the inspection.
A child was seated in a kidney table and the safety strap was not fastened. This was corrected at inspection.
Based on the information found during the investigation, a first aid kit was not in the personal vehicle of the driver when a child was transported to school.
Based on the information obtained during the investigation, the operation did not transport a child in the required age appropriate child safety seat when the child was transported to the public school. NOTE: As of completion of this investigation, another child safety seat has been purchased and is used to transport the child.

During review of the first aid kit the kit was found incomplete. Missing item: 1. Themometer This was corrected when the director placed a thermometer in the kit.
During review of Personnel records the director was found to have 28 out of 30 required training hours. This was cited and TA was given.
The caregiver failed to discard of the broken pencil sharpener leaving it accessible to children.
The caregiver did not notify the parent of the less injury the child sustained while in care.
The caregiver failed to assess the injury the child sustained in class after being made aware.
