A mop bucket with water was found in the hallway accessible to children. This was corrected when the bucket was removed from thhe hallway.

One caregiver did not have the training in abuse and neglect and building and premise safety training.

Two bottles of cleaning chemicals were observed to be stored next to boxed of snack crackers and a jar of jelly

A 2lb roll of ground beef was observed to be open and uncovered in the freezer.

3 children with listed diagnosed food allergies did not have an allergy plan of file and available for review,

The fire extinguisher on one of the vans used was not observed to be secured. NOTE: The fire extinguisher was observed to be in the box and under the passenger front seat.

5 car seats located on both vans used were observed to be expired. Expiration years were 2016,2017,2018,2020,2022 NOTE:Corrected at inspection. All expired carseats were removed and dissposed during the inspection

3 cribs used in the infant room were observed to have loose fitted sheets.

A caregivers purse and uncovers electrical outlets were observed in the toddler room. Uncovered electrical outlets were observed in the pre-k room

Two cribs in the infant room was were observed to have two mattresses in the crib. NOTE:Corrected at inspection, the second mattresses were removed during the inspection.

Two vehicles used for transportation did not have have completed first aid kits. NOTE:Corrected at inspection. Missing items were replaced during the inspection.

In the bathroom of the toddler room, the ceiling was observed to missing a panel.

Emergency medical authorization was not available for 2 children being transported. NOTE:Corrected at inspection, the medical authizations forms were provided during the inspection

Several bottles in the infant room were observed to be unlabeled.

In the infant and toddler rooms. diaper rash creams were observed to be on top of the diaper changing table in reach of children.

In the toddler room on the diaper changing table disinfectant spray and hand sanitizer was observed in reach. In the pre-k room a bottle of disinfectant spray way observed in reach.

2 children being transported did not have parent or emergecy contact information. NOTE:Corrected at inspection,parent contact forms were provided during the inspection

3 gallons of milk dated 4/15, 4/16 and 4/16 was observed to be in the refrigerator. One of the gallon with the 4/16 exppiration date was served to children for breakfast. NOTE: Corrected at the inspection. All expired milk was disposed of during the inspection.

An emergency evacution diagram was not posted near all exits used. NOTE: Corrected at inspection, diagrams were posted during the inspection.

A television on top of a rolling shelf was not observed to be properly anchored.

A cleaning solution Lemon Plus was not being used according to manufacture instructions. According to the director, an unknown amount of solution is mixed with a bottle of water. The solutiion is used to sanitize table tops According to the instrcutions, the product is not to be used on surfaces that come into contact with food.

3 children observed sitting in a bean shaped table with straps were not observed to be strapped in. One of the seats did not have straps.

A 6 mo old not yet on table food did not have feeding instructions completed by the parent

The children product safety certificate was last completed 2/1/23

The child safety alarm on one of the vehicles being used for transportation was not operational during the inspection NOTE: The operation was informed the van may not be used until the safety alarm has been repaired

A catipliller toy on the playground was observed to be broken and had exposed shear points.

Two nap mats were torn, exposing the underlying foam. This was corrected at inspection when the Director disposed of the torn mats.

Two infants did not have current infant feeding schedules.

All six of the classrooms did not have activity plans available.

One of four children records did not have a current immunization record.

Two of the bucket seats at the toddler table did not have safety straps. This was corrected at inspection when the Director removed the seats from the classroom.

The floor in the infant room was sticky and needs to be cleaned.

The diaper changing mats in the infant and toddler rooms were torn, exposing the underlying foam.

A caregiver did not know how many children she was supervising during naptime.

A caregiver was using a personal cellphone while caring for children.

An11 month old non-walking infant in the toddler room was asleep on a mat. Note: This was corrected when the director placed the infant in a crib.

In the afterschool room, there were cleaning supplies located near the handwashing sink. Note: This was corrected when the director moved the supplies to a higher place.

On the playground, there are two pieces of wood missing from the fence.

The loose-fill surfacing did not measure 9 inches. The highest playing platform is greater than 5 feet. This must be corrected by the date listed.

Nap mats were inspected and lacked the children's name.

One caregiver in the preschool classroom was lacking the required background check.

A two month old infant was observed sleeping with a bib. The infant was also sleeping on top of a boppy inside of the crib.

A 2 month old infant was observed sleeping in a swinging seat with a blanket. -Pictures were taken and the infant was removed from the seat.

The operations CPSC form has not been certified since 2/21/20.
