Upon arrival, it was determined a qualified caregiver was not left in charge in the directors absence.
Cleaning supplies were stored under a cabinet in areas accessible to children. During this inspection I observed a child in the Preschool class open a cabinet containing cleaning supplies.
During this inspection a mirror was observed in the crib of a five-month-old.
During this inspection the children in the PreSchool class was observed fighting and running around the classroom.
During this inspection I attempted to enter the Toddler I classroom however the door was blocked, preventing entrance into the room.
A caregiver in Toddler I classroom was unaware of the number of children responsible for. The caregiver stated twelve children were present however thirteen were counted in the room.
During this inspection the sign in and out information via the ProCare app was not immediately available for review. Once reviewed the sign in and out log it showed the following: None of the children in the Senior Infant room were signed in. Eight of thirteen children in the Toddler I class was signed in. Eight of fifteen children in the preschool room was signed in. The sign in and out log also showed eleven school age children were signed in but none were present at the operation.
An eight week old infant was observed laying on their tummy. The infants caregiver stated the child was not able to completely roll over without assistance.
During today's inspection I observed the preschool caregiver forcefully grab a child by the arm.
One infant caregiver was observed caring for five children ages 8 weeks, 5 months, 5 months, 5 months, and 10 months. The specified age group is 0-11 which allows the caregiver to supervise a maximum group of four children. The caregiver was over ratio by one child. The caregiver in the Toddler I classroom was observed caring for 13 children. The specified age group for the classroom was 2 years old which allows the caregiver to supervise a maximum of 11 children.
Based on the information obtained during the investigation, the caregiver failed to put a new diaper on a child after changing him.
During walk through it was observed infant was sleeping in the bouncer. Note: Caregiver immedaitely removed the infant from bouncer and placed infant inside the crib.
The fire exthingusher inside the bus transportation was laying on the floor behind the driver/passeger seat, and not mounted on the passenger side Note: Correction were made at inspection.
There is chipped Paint on the wall inside the Schoolers Room 116. (Corrections) The Chip Paint on the wall was made during inspections.
