Records were evaluated for ten children enrolled and the following was determined: Nine children needed an address for the emergency contact listed. One child needed an emergency contact other than the parent. Four children needed a food allergy emergency plan signed by parent and physician. Four children were in need of a special needs statement. One child needed a health statement. One child needed an updated immunization record.
