During the inspection I observed that a fenced playground yard did not have two exits.

During the inspection it was found that two children were on a field trip without their food allergy emergency plans and allergy medications.

During the inspection I observed three medications to be expired for two children in care. One epinephrine injection has been expired since July of 2022. One auvi-q injection has been expired since 8/28/2022. One bottle of benadryl has been expired since 04/2023.

A review of the Employees Who Left Employment in 2022 data from February3 ,2023 revealed that the center had not reported the number of employees who left employment in 2022 as required.

The staff training records were not available for review by Licensing.

In the infant classroom the SAG is 0-11 months, there was one caregiver in the room with 8 infants when one of the caregivers came to the door.

The child's file has not been updated since enrolled as an infant, he is know a school age child.
