In two of the toddler classrooms the diaper changing mat had several tares in the middle of the changing mats. NOTE: Corrected at inspection when the two changing mats were replaced.

In the toddler classroom it was dark when I walked in the classroom where it was hard to see the children. Corrected: When the caregiver open the blinds in the classroom.

A three month old infant was sleeping in a bouncer. Corrected: When the caregiver took the infant out the bouncer.

A caregiver stated she had 10 children, when I counted they were 11 children in care the caregiver was unaware of how many children she had during nap time.

Two caregivers one in the infant class and one in the toddler class observed providing direct care to the infants in these classrooms prior to receiving eligibility status.

The caregiver was unsure of the ages of the children she had in care.

The toddler one class had 6 children in the group with one caregiver SAG 12-17 month of. The ratio for this class is one to five.

The caregiver used harsh punishment when she forcefully sitting the child down in a chair.

The caregiver didn't show good judgment when she yanked a child out of his seat and dragged him across the room.

In the infant classroom the specified age group is 0-11 months, the operation had 12 infants in the classroom two over the ratio size.

The operation did not have how they would evacuate or relocate children under the age of 24 months in their emergency preparedness plan. Corrected: When the director wrote in in the emergency preparedness plan.

A child was able to leave the operation without supervision.
