During inspection of the outdoor play area the exit door to the playground was found to not close easily. Door latch was found to be grinding when attempting to pull closed.

One file did not have the health statement or the parent statement that the child has been seen by the doctor in the last 12 months and is healthy enough to be in school. One of the files did not have the home address listed. One of the files did not have the health care information. Two of the files did not have an admission date. Three of files have not been updated in over one year.
