The inspector reviewed the facilities list of children that are normally transported each day. The inspector reviewed the binders of enrollment forms that are kept in the vehicle while transporting. The inspector observed that the binders were missing forms for 5 children that are normally transported. These forms would contain their emergency medical information. NOTE: this was corrected at inspection. The director made copies of their enrollment forms and put them in the vehicle binder.
