The emergency drill log was reviewed and shows that shelter in place, severe weather, and lockdown drills, each required to be conducted four times per year,were all documented as being completed on the same day. Additional dates listed in the log do not specify which drill type was conducted. Fire drills from May 2025 through November 2025 were all documented as occurring at 3:30 p.m., with the evacuation time recorded as 3:30 p.m. to 4:00 p.m., indicating no variation in drill times. Additionally, there is no documented fire drill for the month of December.

Based on the information obtained during the investigation, there have been at lease 2 occasions where the driver left a group of children 5-9yrs old unattended to go inside of a school and pick up a child who had not been released on time by the school.
A caregiver was observed to be asleep while supervising a group of 12 5y-12yr old children during naptime. The caregiver denied being asleep, a picture of the caregiver sleeping was shown to the caregiver and director.
The playground fence was observed to have several broken and warped boards.
4 anchoring bolts were not observed to be below playing level. The bolts created a tripping harard for children.
Upon arrival, a group of children between 10m and 18m were observed watching TV. The TV was not turned off until the caregivers were instructed by the director
For the training period of 2020-2021 the director did not have 6 hours of management training
A group of children was observed waiting for a long period inbetween activites. Upon arrival the director stated the children just finished snack. The children were observed sitting and waiting for 28 minutes before being given blocks
Records for 10 children currently enrolled was reviewed. 4 children did not have an address listed for the emergency contact. 1 child did not have a special needs statement completed.
The driver did not have transportation training available for review
3 infants under 17 months were observed to be combined with older children. The youngest child in the group was 10 months old, the oldest was 8 years old.
A caregiver with no prior experience who was hired on 4/29/2021 did not have the completed 24 hours of pre-service training. NOTE: Only 8 hours of pre-service have been received
A toddler slide was observed to be cracked and had an opening about 5 inches long. NOTE: The slide was removed from the playground
Annual training was reviewed for the director for the trainiing period 2/2/20-2/2/21. Only 24 hours of training was obtained during the training period
During the inspection it was learned that for time out children sit in a corner and are made to face a wall or corner. Additionally, children remain in time out for a minimum of 5 minutes and maximum of 30 minutes.
Two climbing structures on the playgound were observed to have loose-fill surfacing less than 6 inches deep. NOTE: Grass was observed growing through the mulch.
