Training records for 9 staff including the director was reviewed. All staff received more than 46-64 hours of training within a 24 hour period. Training certificates were copied and whited out to add a new caregiver's name. Original certificates were not available. Some of the certificates obtained for the director listed the director as the trainer and participant.
6 children's records were reviewed and observed to be mssing the following documentation: 3 were missing completed emergency contact 2 were missing contact information for release persons 1 was missing the child's home address 1 did not have emergecy medical authorization.
Nap mats used are not labeled with the child's name or number. According to staff, mats are placed down and sleeping spots are identifed once the child's blanket is placed on the mat.
Several exits doors designated as emergecy exitss and evacutation routes were missing a posted diagram.

The 2 year old classroom was observed to be overratio. There were 26 children between 21m-2yrs old. The Sepcified Age Group is 2yrs old. The classroom was over ratio by 4 children
Documentation of manufacture instructions for a climbing structure used for the 2 year old group was not available during the inspection. According to the manufacture, the equiment is designed for children 6-10 yrs old
Three out of five children record evaulated needed a health statement.
A caregiver in the infant classroom was observed using a personal cell phone. Note- This was corrected during the inspection when the caregiver put the personal cell phone away
