The operation is not notifying parents of less serious injuries.

The proper level of supervision was not maintained. The caregivers were unaware of how the child incurred the injury.

The caregiver did not demonstrate good judgement with the inappropriate joke of stating she bit a child in care.

The proper diaper changing steps were not followed when a child's diaper was only changed once after being in care for over 8 hours.

During the review of children's records 3 out of the 10 records evaluated was found to be missing required information for the emergency contact person.

During the review of children records 2 out of the10 records evaluated was found to be missing the food allergy emergency plans.

During the walkthrough I obseved 1 caregiver caring for 12 children with a SAG of 2 years. The cargiver was over ratio by 1. This was corrected when the director divided the children into two classrooms.

Caregiver is completing janitorial duties in the bathrooms during naptime and while counted in ratio. Caregiver failing to intervene to stop and prevent inappropriate child to child touching.

During the walkthrough a caregiver stated they were caring for 12 children, I counted 14 children in the classroom.

During revview of the centers operational policies, the policies were found to not have included promotion of physical activities.

During review of Fire Safety the operation was found to not have a current fire inspection as required by minumum standards.

Although the operation conducted a fire drill for the month of September there was no documentation. This was corrected when the director completed the documention.

During the walkthrough I observed a 6 month old asleep in a restrictive device for approximately 15 minutes. This was corrected when the caregiver removed the child and placed the child in the crib.

During review of personnel 2 out of 10 records evaluated did not completed all 24 hours of required annual training.

During review of records 1 out of 10 records evaluated did not have an allergy plan for a diagnosed allergy listed.
