In the emergency preparedness plan it did not state how the operation will evacuate children under 24 months out of the building.

Four caregivers were observe to be vacuuming the floor while children were eating lunch in the pre-school, toddler, infant classroom. In one of the infant classroom the caregiver was vacuuming while infants sat in the high chair and bouncers.

Caregivers did not know the age of the children they were caring for.

The director did not have 30 annual hours for training year 8/18/21 to 8/18/2022, they were only 15 hours. I took 15 training hours from training year 8/18/2022 to 8/18/2023.

The operation did not have in their operational policies about safe sleep.

The children were observed to not wash their hands with soap and water before eating lunch.

An caregiver dose not have qualifications for having a high school diploma.

Two caregivers did not have in person CPR training.

The operation did not have in their operational policies on suspension and expulsion

In the first infant classroom the caregiver had 5 children SAG 0-11 months old, they were one child over. In the second infant classroom it was one caregiver with 6 children SAG 0-11 months they were two children over. In the three infant classroom they was one caregiver with 6 children SAG 12-17 months, they were one child over.

After further review it was determination that the operation was not allowing parents in the building.

It was observed infants sitting in restrictive device and high chairs for over 15 minutes and not given the opportunity to get on the floor and play.
