Health inspection for this operation expired since 2/02/2025.

Licensing did not observe any current fire inspection for this operation. The one on file had expired since 1/29/2025.

Based on DFPS investigation operation did not notify parent at the time of pickup of the situation that placed their child at risk.

There is enough evidence according to DFPS investigation that a 20 months old child was neglected when child exited the playground and was found in the operation parking lot. An administrative penalty will be recommended as a result of this citation per HRC &42.078.

Based on DFPS investigation caregiver did not know the number of children she was responsible for when a 20 months old child exited the playground to the operation parking lot.

A 20 months old child was able to get pass the gate from the playground into the front parking lot next to the playground.

Incident report has not been signed by parent.child comes only twice a week.

First van was missing scissor and tweezers and the second van was missing scissors.

Few cots were not labelled or numbered during naptime. This was corrected during inspection.

I observed that one four year old did not wash his hands before eating snack. This was corrected when child was asked by the director to wash his hands.

Part of the outdoor play area has grass that needed to be cut. Picture was taken.

Operation does not currently have Sanitation inspection.
