There was 4 month old infant left in a playpen while a caregiver was in a seperate room.

There was a 5 month old infant observed sleeping on her stomach while in the crib. The caregiver in charged stated the infant was not able to roll over on her own.

During the follow up inspection, the smoke detector testing was still not documented. This was corrected when the director tested the smoke detector and documented on the form.

During the follow up inspection, there was still no documentation showing the carbon monoxide detector had been tested. this was correced when the carbon monoxide was tested.

The fire inspection on file expired on 8/9/2022.

There was no caregiver in the classroom with the mixed age group during naptime. A caregiver, left a 3 year old child alone in a classroom.

The caregiver did not know the age of the children she was carig for in the mixed age group classroom.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

During inspection 2 toddler classrooms were observed to not have labels of children name to store belongs.

During inspection in play area broken pieces of wood with nails exposed and 4 live ant beds was observed.

Director fail to ensure a caregiver left in charge of a infant group of children without the required orientation, pre-service training, SIDS, Infant, and CPR/ First Aid. Caregiver hired on 6/28/2021

Orientation documents wasn't available for review during the inspection.

At arrival the school age caregiver left 2 school age children unsupervised to answer the door for Licensing.

There were 4 kids in the Toddler 1 class under age 2 observed watching tv.

Caregiver did not know the age of children in the Toddler 1 classroom.

3 out of the 5 caregivers did not have the full 16hrs of pre-service training. 1 caregiver had 6hrs and 2 caregiver had 9hrs of pre-service training.

At the time of inspection daily menu was not available to be observed.
