During the inspection 3 caregivers provided an incorrect number to licensing for the number of children they were responsible for supervising.
An infant was observed sleeping in a restrictive device at 12:51pm the infant dailly report showed the infant went to sleep at 11:40 am. The infant was asleep in the swing for over an hour. The infant was removed from the swing.
A caregiver was seen using her personal device to watch a show during nap time. This was corrected at the inspection when the caregiver put her phone away.
The fence around both play area was only 3 ft and 9 inches, it is not the required 4ft tall.
Two infants did not have updated feeding instruction forms the forms was last updated in September.
The sanitation inspection expired 10-4-2023.
On the infant/toddler playground they is only one exit, the fence area needs another exit.
Under the swings and moving equipment they did not have the required loose-fill surfacing under the equipment.
On the playground the grass area was high and one of the play structures had broken pieces.
In the school age classroom they was one caregiver with 48 children in care they were 22 children over ratio. In the infant classroom the caregiver had 5 infants specified age group 0-11 months old the caregiver had one over ratio. One caregiver had 19 children specified age group 4 years old, the caregiver had one over ratio.
The director and three caregiver do not have annual transportation training. One caregiver last training was done 3/6/2020.
The director did not have any training hours for training year 2/15/2021 to 2/15/2022. Corrected: When the director had training hours for current training year 2/15/2022 to 2/15/2023.
Two caregivers are missing first aid training one caregivers expired 1/27/2019 and the other caregiver did not have a card.

I asked a caregiver how many children they had it was stated 25, when I counted the children it was 26.
The pre-school and school age children were combined together in the school age classroom. When asked the school age caregiver how many it was stated 15, when I asked the first pre-school caregiver how many she stated 6. When I counted all the children in the room their where 26 children, which left 5 children unattended without a caregiver.
Children were sitting around at the tables and carpet with no activity sitting and waiting to go home.
When asking a caregiver how many children they had it was stated 15 and when I counted they were only 13 children. When asking another caregiver how many children she had it was stated 17 when I counted it was only 16 children.
The school age caregiver had 27 children, the max group for one caregiver is 1 to 26.
During the determination of the investigation it was stated that parents are not allowed in the building to drop children off or talk with caregivers. Parents are only allowed at the front entrance of the operation.
During the inspection I observed a changing table that was found to be contaminated with food and other substances. Photos were taken. This was corrected when the caregiver sanitized the changing table.
During the inspection I observed a caregiver wiping two children's noses without washing her hands in between.
